MAINE: Youth Referral and Tracking Toolkit

The Youth Suicide Prevention Referral and Tracking Toolkit helps schools track and assist youth identified and referred for suicide risk. The toolkit was developed by the Maine Youth Suicide Prevention Program, which found that schools with poorly coordinated data collection often lack clear guidelines and protocols for staff to follow if they are concerned about a student’s risk for suicide. In addition to tracking forms and tips on collecting data, the toolkit includes guidance on how to use early identification, referral, and follow-up data to improve a school’s broader suicide prevention efforts.
