This sentinel event alert is intended to help organizations that provide both inpatient and outpatient care better identify and treat individuals with suicide ideation. The alert replaces two previous alerts on suicide (issues 46 and 7). It covers suicide ideation detection, as well as screening, risk assessment, safety, treatment, discharge, and follow-up care of at-risk individuals. It also includes suggested actions for educating staff, keeping health care environments safe, and documenting care.
